We are in-network with all major insurances, including Medicaid, Medicare, Cigna DHMO, Tricare and ADDP. We also accept secondary insurances and will file claims on your behalf. We try our best to determine your insurance eligibility prior to your appointment, for your convenience. We ask all new patients to provide their insurance information while making the initial appointment.

J-Plan: In-House Plan

The rates shown below provide coverage for a fiscal year beginning on the date of payment. (e.g. May 1-May 1)

$399 for Individual
$699 for Individual & Spouse/Partner
$799 for Family of Three
$899 for Family of Four + $100/year for additional family members

The J-Plan works like most other insurance, and covers the patient for the following:

100% coverage of preventative care twice a year(incl. regular prophylaxis cleaning, exams, x-rays, and fluoride for children under 18)
100% for limited (problem-focused) exams (incl. any necessary x-rays; excludes 3D CT scans)
50% of all other services (excluding treatment from Specialist Doctors)
20% of all Specialist Doctor fees
